DISCLOSURE REGARDING PAYMENTS:

CHANNEL® Integrative Health & Rehabilitation is a "cash-based" clinical practice that does not accept traditional insurance plans or Medicare/Medicaid.  We do accept cash, checks, and all major credit cards (including Health Savings Account cards). You are free to submit your receipt, clinical note, or any other supporting documentation to your insurance provider for potential reimbursement.

CANCELLATION POLICY:

Because CHANNEL® Integrative Health & Rehabilitation subscribes to a individualized service, we kindly ask that you cancel your appointment no less than 24 hours prior to your scheduled time (when applicable).  This will allow us to make the necessary adjustments and/or make arrangements with other clients.  Cancellations made less than 24 hours prior to your scheduled appointment will result in a 50% charge of your planned service fee. 

CHECK-IN:

Please arrive at least 15 mins prior to your scheduled appointment time.  This will provide you the opportunity to decompress and prepare yourself for your session.  

MEDICAL CONCERNS:

As a new client of CHANNEL® Integrative Health & Rehabilitation, you will receive a general health/functional assessment as part of your first session.  If your health/condition changes at any point in time or if we are not able to adequately address your needs, we will refer you to your primary care physician or another healthcare professional.
